There are three forms of concealed handgun carry systems operated in the United States. Constitutional Carry, Shall-Issue, and May Issue.

Constitutional Carry came from Vermont, where anyone over 18 not prohibited from carrying a handgun may do so, openly or concealed, no permit required. This has been the law there since the state was founded in 1781. Complete Constitutional Carry has been added by the following states: Montana (1991), Alaska (2003), Arizona (2010), Arkansas (2013), Kansas (2015), West Virginia (2016), Idaho (2016), Mississippi (2016), Missouri, New Hampshire, and North Dakota ( 2017). Constitutional Carry for state residents has been added for Wyoming (2011), and partially for certain types of carry in Texas (2007)

"Shall Issue" means that if you fill out the form and (if required) take training and/or marksmanship classes, the state shall - is required - to issue a permit to anyone at least 18, who is not prohibited from carrying a handgun. The big shocker here was since 2008, when the U.S. Supreme Court struck down the essentially "no issue" regeme in Washington, DC in the case of District of Columbia v. Heller (2008). Since then, DC has tried to add restrictions. The last one, a requirement that an applicant for a permit have a good reason, was struck down earlier this year in the DC Circuit Court of Appeals case of Wrenn v. District of Columbia. As such, this makes DC a "shall issue state" in which, if you're not disqualified, if you fill out the form, pay the fee, and take the training class, they have to issue you a handgun permit. The other states that are "shall issue" are:

(Limited discretion to deny permit): Alabama, Arkansas, Colorado, Georgia, Illinois, Indiana, Iowa, Minnesota, Missouri, Montana, New Hampshire, North Dakota, Oregon, Pennsylvania, South Dakota, Utah, Virginia.
(No discretion to deny permit): District of Columbia, Florida, Kentucky, Louisiana, Michigan, Mississippi, Nebraska, Nevada, New Mexico, North Carolina, Ohio, Oklahoma, South Carolina, Tennessee, Texas, Washington, Wisconsin.

"May Issue" means the state can decide whether or not to issue a permit (and in some places it is essentially a "no issue" state unless you have political connections): California, ยท Connecticut, Delaware, Hawaii, Maryland, Massachusetts, New Jersey, New York, Rhode Island.

Now, Maryland has the same "you need a good reason" requirement to get a wear and carry permit, but given the decision in Wrenn I suspect that this requirement will not stand up to judicial scrutiny. Thus I expect Maryland to shortly become a "shall issue" state the way DC has become.
