Quests are activated instantly by motherfuckerfoodeater 10/24/2009, 8:14pm PDT
All of them require going to specific places, though, so it shouldn't be hard to avoid doing them. The level cap is raised instantly, and Broken Steel means tougher random encounters regardless of where you are in the game.
