At the end of 3 you can save her father or not save her father. If you save her father, the whole timeline is altered and you wind up in the cool kids club, her parents stay together, she ends up being paralyzed in a car accident, wheelchair bound with a tube in her throat, an expensive hospital setup in her house where the garage was, all of her friends abandoned her, and her parents are in so much debt they're going to lose the house.
In chapter 4 you find out that her body has atrophied so much that she's going to die soon because her lungs are going to fail. She asks you to help her commit suicide. If you do (and also I'm assuming if you don't), then you have exactly one unavoidable option to immediately time travel back through the photograph and then you watch a cutscene where you decide to let her father die and send the timeline back to normal.
